@Twosox Thanks for sharing that link! It's a neat service to turn a single paper form into a Canvas mobile app form. It's great if you were going to stop collecting paper information, and switch to a mobile. It's perfectly complementary to Captricity's service, which serves the case of you having a stack of 1000 filled forms -- Captricity turns that stack into a spreadsheet and can send it into your database.
